Sample and form must match, particularly with respect to patient's details.

If the sample is for blood products, most centres will require additional measures. Specific arm band to be placed on the patient with specific number / bar code. Same bar code / number in notes, on sample and on request form. This bar code / number will come down with the blood products and needs to be checked against the patient's arm band prior to administration.

I'm not familiar with what happens during the test, really. That's all just up in the labs. I know the sample and form get a lab number put on them but beyond that you'll have to get a lab tech to help you.

Before ---name,age,gender,address,and consent.
During ---speed of flow,appearance of structure,reaction of the patient
After--results ,normal range ,normal average in that laboratory.
