I babysat, have 3 kids and will only have made $15,000 by years end. Can I still claim the E.I.C.? How would I go about claiming what I earned?

If you have at least one qualifying child, you'd be eligible for EIC. You'll claim your babysitting income on a schedule C, fill out a schedule SE to calculate self-employment tax (social security and medicare), then the numbers from the bottom of the two schedules will transfer to a form 1040 to figure your tax and your EIC.

Yes.

Form 1040 (Long Form) along with Schedule C and Schedule SE.
