She is 3 months old and still has accidents in the house. She will pee not long after coming inside sometimes.
We punish her when she goes inside and also offer positive reinforcement when she goes outside...

I read a VERY good book on training a dog to be housebroken.
Positive reinforcment is the best thing for a dog. but you can also teach your dog to go on command (sounds bad but very good for them..better than holding it)
ok here are the basics:
1.while walking to the front door with your pup continuously say "potty" or "out" you'll feel stupid for saying "potty, potty, potty" but shell soon learn when you say potty, she needs to be at the door!
2. once outside when you pup FINALLY goes, continuously (only while they are going) say "pee, pee, pee" and "poo poo poo" (or any word you want to you but make sure you use two different words for peeing and bm's)
3. when your dog is finished not only say "good girl/boy" give them a treat
4. VERY IMPORTANT do not go indside IMMEDIATELY after he or she does there business. they are smart and they will learn that when they go potty the fun of being outside is ended and they will wait and wait and wait to potty so they can be outside longer. if you let them play afterwards they'll go as soon as they get out!
-soon they will learn they are going to get more than a pat on the head for going out and after about of month of repetitions they will automatically potty when you say "pee" or "poo"
It really works it worked with mine ina month she was trained by 4 months old NO PROBLEM. (also a kennel while you are away helps them to get the structuring down bc they wont generally potty where they live and soon they will learn the whole house is where they live, not just the kennel)

Your puppy is still very young and it's not realistic to expect a 3 month old not to have accidents. No more punishing, if you catch her squatting, scoop her up, tell her no (no shouting, calmly) and take her outside and say, go potty - it's probably too late, but do it anyway. Keep praising her. By the way, clean her accident thoroughly with an enzyme cleaner, Nature's Miracle is a good one. Good luck.

Dont worry,she is still very young and her bladder muscles are not well devolped. Just stay consistent, praise the good behavior and don't punish her for the accidents. Its like a toddler, you can't punish them for having accidents. By the next 2 months, she should be trained. Also if you notice a pattern, then make sure and take her out...after she wakes, after she eats, after naps, after playing, etc.

Stop punishing her for going inside. It's your fault. Stay outside with her until she goes. Period. End of story. Reward her for going outside. At three months old 4 hours is about all she can hold it. Put her on a schedule and STAY outside with her until she goes.

12 weeks is still too little. Don't punish the accidents - instead catch her in the act and take her outside the very moment it happens. Crate train her and crate when you can't supervise fully.

It could take up to one year for the dog to be fully trained.

just like with babies that teeth for two years they say that a dog has its terrible two too..good luck and take the dog to an obidient school..they work

yeah the best thing to do is to go outside with her and once she goes potty reward her with a treat and petting her, if she does go potty even after being outside punish her by tell her no and rubbing her nose in it and putting her in her kennel, and if she doesnt have a kennel I highly recommend getting her one..
