It seems like this is always true but I wanted to ask and make sure how come the last letter in so many Italian last names are a vowel.

It appears you are correct.

All Italian nouns end with a vowel; the same is true for almost all surnames, but not for all: for instance De Magistris, Badoer, Berlinguer, Manin. Not very used words, such as surnames, have less language "evolutive pressure".

Not certain if there isn't right off the top of my head. However, not all names ending in a vowel are Italian or Spanish. We have had some trollers on this board who apparently think so.

It's true for the vast majority of Italians, but it's not "always true"; lot of people do have a last name that end with a consonant, mostly people from Veneto but still from all over Italy.

not everywhere in italy
i live in sardinia region, here many surnames end with the letters S, an also with X and R

First of all, there is no such thing as a full-bloodied anything.
Second, bloodlines has nothing to do with names.
If you are referring to those people who speak Italian, then yes, most do.

all the full blooded italians last name ends with a vowel.
my husband is 100% italian born in italy came here when
he was 12 years old. our last name ends in a vowel.
