Hi. NO...people used to do that way back, but it's discriminatory now. Don't send one.

Not if the position is in the US. Employers can't discriminate against you based on gender, age or race and a photo will give up those characteristics about you.

It's also not appropriate for an employer to ask for a photo of you with your resume/CV.

No, I don't think so. Employers aren't allowed to discriminate, and don't want to discriminate. Let them judge you on your experience and qualifications, not on the visual characteristics.
