Could a car accident result in severe farsightedness aka hyperopia?

or what type of accident would/could result in it?

Yes, just.

If the lenses of the eyes were dislocated by the trauma, the person's Rx would suddenly jump to +12.00 or more.

There would probably have to be an underlying weakness, though, for it to affect both eyes.
(Marfan's syndrome, Homocystinuria or other related genetic anomaly: research ectopia lentis)

Any kind of head trauma could cause that, espicailly a MVA.
