When filling out "name" section of the AKC registration form, are you supposed to include the dog's last name?

Or is that assumed since we are giving our last name in another section of the form? If we only include the first name (Charley), will that be the only name that shows up on the registration? We want his full name to be there I think.

I've looked at the AKC site ( http://www.akc.org/reg/namingofdog.cfm), but don't see anything there. Hopefully someone who's been through this can help.

Also, any good middle name ideas for Charley? He's a boy.

Good question why. He's a miniature schnauzer. Our last name is Holley, so I'd like the middle name to be extra clever since the first name isn't (named after family member).

Whatever you put in the name section is what your dog's name will always be. AKC won't let you change it once registered even if the dog goes to someone else in the future. The registered name is permanent so make sure you make it what you want to begin with. If you want him to have your last name then you must include that when you put his name on the paperwork. You are allowed just so many letters and spaces so make sure it will all fit also.
It would be easier to help you with a middle name if I knew what breed Charley is.

See, generally, when a dog is purchased from a good breeder, a kennel name is used. It starts the name. Like if my kennel name is "Redwood", he could be Redwoods Charlie. Clearly, where ever you got the dog had nothing like that. So, you have to come up with your own. Naming him something like Charlie Jones would be kind of lame. You could call him something like "Good Grief Charlie Brown"...or some other phrase that uses Charlie in it. Think of something interesting.

You are not required to use the "call" name in the registration name at all.
You can think of something totally unique.
I have Torbay's Jamacia Me Crazy, and we called him Ziggy...after Ziggy Marley.
You can think of something with Holley in it and still CALL him Charlie or Charley.
You could call him Holly's Deck The Halls, and call him Charley.
The sky is the limit. It is NOT like naming a child. You do not have to have first, middle and last name.

Middle name help:

Charley Boughs of Holley (Like in the song - Deck the halls with boughs of holly)
Or Charley Bows of Holley (or Bowwows of Holley)

The dog name has to be somewhat unique for the breed. I think only 20 dogs of same name per breed are allowed, so people come up with long first, last and middle names for their dogs.

For example, my dog's sire was named Angelic Cafe Mocha and called by the name of Mocha.

Yes you will need to put all three names on it
It will come back with the name you put
If you want it to have your last name then put it like this
William Charley Brown
