The Chairman of the Joint Chiefs of Staff (CJCS) is by law the highest ranking overall military officer of the United States military, and the principal military adviser to the President of the United States. He leads the meetings and coordinates the efforts of the Joint Chiefs of Staff (JCS), comprising the Chairman, the Vice Chairman of the Joint Chiefs of Staff, the Chiefs of Staff of the United States Army and United States Air Force, the Chief of Naval Operations, and the Commandant of the United States Marine Corps. The Joint Chiefs of Staff have offices in The Pentagon
ALL these officers will be at the paygrade of (O-10).
NOTE though, that the Joint Chiefs are PURELY advisors to the President and have NO OPERATIONAL Command over combat or deployed units.

While he is The Commander In Chief of the military, the President is not actually in the military. He is a civilian. The highest ranking military officer is the Chairman of the Joint Chiefs of Staff, currently Navy Admiral (four stars) Mike Mullen.

The highest rank ever is the 5-star general. I don't believe any exist right now. Eisenhower was, I believe, the last one. The 5-star exists only during wartime but I don't believe one has been named (I could be wrong).
That leaves four-star general (and equivalent in Navy). While there are several of those, one is the highest in each service and is so named. For the Army it is the "Army Chief of Staff", for Navy "Chief of Naval Operations", for Coast Guard "Commendant of the Coast Guard", for Marines "Commendant of the Marine Corps" and for the Air Force it is "Air Force Chief of Staff."
These five people comprise the Joint Chiefs of Staff and they are presided over by the Chairman of the Joint Chiefs of Staff and that by the President, the Commander-in-Chief.
