A passenger is going to Perth. But his status is 'stateless' (maybe from old system error from his birth certifcate) and holding a Brunei PR. But traveling from Kuala Lumpur (malaysia). Does he needs to apply visa? how?

Perhaps it is a "Certificate of Identity"? This should be treated as a passport by Australia, but he will almost certainly need a visa. However, Australia sometimes issues these documents themselves, so there may be some special arrangement.

Does it say who issued the document?

He should apply in the same way as someone with a regular passport - through the nearest Australian embassy.
