My grandpa was diagnosed with osteonecrosis about 6 months ago. Still nothing has shown on x-rays, MRI or bone scan. Right knee has horrific pain - can't walk - How long does it take for the bone to die & does it ALWAYS make new bone? Is there only ONE episode of pain or does it continue in pain FOREVER or until joint replaement?

Osteonecrosis remains painful till sequestrum is formed. Sequestrectomy can be done to relieve pain. Sequestrum forms in eight weeks.

I'd love to help you but I'm afraid I'm a bit confused here. How was he "diagnosed with osteonecrosis" when the roentgenographic and MR studies showed nothing? With osteonecrosis there would be high T2 signal quite early on... and now six months down the road you say that there's no imaging evidence for osteonecrosis. How does one make a diagnosis of osteonecrosis in the absence imaging evidence to corroborate this? Could it be that your grandpa has osteoarthritis as opposed to osteonecrosis?
