The county (or parish) auditor's office will tell you what steps you have to take. It's different for each locality or state. You will need some information from your most recent tax return, as well as information on additional income you may have gotten that was not taxable, such as Social Security.

I do tax returns for a living, and there are clients who bring me their form each year. It takes me about 5 minutes to fill them out, which I do at no charge. If you have your income taxes professioanlly filed, they might be willing to offer this service as well.

Call your county office that deals with property taxation and ask them. There is probably a form to fill out. Some places they mail them out to everyone.
