Simple, you have a doctors appointment every week at such and such a time. Don't offer anymore info then that, they will be supportive and try to offer help.

Tell them the truth, but not the whole truth. Basically, you had a doctor's appointment. Nothing more, nothing less. If they ask something like 'what's wrong with you', just say something along the lines of 'nothing serious, just have to go for regular checkups', which isn't entirely wrong.
If you want a believable lie, slight scoliosis(curvature of the spine). In slight cases it is not noticeable, and is more common in this day and age especially because of shoulder bags etc. In fact, I carried a backpack my entire school life and I have a slight curvature. My younger brother has it as well, only slightly more curved(not enough to wear a brace though), and he had to go for checkups every two weeks till they thought he wasn't growing much any more, and so it wouldn't worsen.

Appointments with a Chiropractor for back pain or headaches. These often take place 1 to 3 times a week, and treatment can go on for years.
And it's a pretty mundane, boring explanation of why you are late.

My 10 y/o daughter has a similar problem as she is being treated for depression.
She has told her close friends why she is sometimes late for school and they have been a great support to her.
With other kids she just tells them she is seeing a Doctor for some of the symptoms she suffers ie headaches & stomach problems. That way she doesn't feel like she is making up stuff.
Hope this helps & good luck with your treatment

First of all, you should be proud of yourself for seeking help. Take are of yourself. I was going to tell you don't worry about your friends and if they were your real friends, they would support you. However, I know what its like to be seeing a psychiatrist. I know that not everyone is understanding and some might turn against you. It has happen to me. I would suggest to try to talk to your friends one on one and do it when you feel ready. It sad to think you might have to lie to protect yourself, but at least you are doing the right thing in seeking help. Take care.

you dog walk / sit. tell them you have to go by grandma's house to help her out. or you donate some time at a senior citizen home, because it makes you feel good. make sure you tell your parents, so they don't blow your cover. you shouldn't have to tell your friends anything so personal.
