though i do not have a precise answer, perhaps i can clarify further. the term cardinal probably originated from late latin cardinalis, which means principal or important. The early cardinals were important priests who assisted the pope in various matters. It is in the middle of their name most likely due to tradition, as well as the all encompassing nature and importance of their role.

I've always imagined a comma, as Cardinal is a title. Sort of like "Alfred, Lord Tennyson".
