I would say that headaches without other symptoms are not the norm for 4 year olds, but they are also not unheard of. An examination is in order. Take him to a GP, an evaluation of his vital signs and neck rigidity are needed. IF persistent or recurrent an MRI is not uncalled for. What I would not recommend, especially in a child, is cranial manipulative therapy, either by an osteopath or a chiropractor. If the GP can not clear him, see a pediatric neurologist, before you have anything other than paracetamol.

Well a headache can happen anywhere in the head.
I would keep an eye on it though. My six year old son had a stage of having headaches when he was 4. He had numerous visits to the Paedeatric unit at the local hospital. They then arranged to have a MRI scan of the head, to rule out any major causes.
Nothing was found and he grew out of it.

Keep an eye on it. My daughter was 4 (almost 5) when she came in with a bad headache and I gave her Tylenol and she went back outside to play. 30 minutes later she was saying it was worse. I made her relax and lay down for a while, 2 hrs later she was a LOT worse and she started throwing up and saying her neck hurt. I took her to ER and she had meningitis!! VERY Scarey!!!!

YES... its unusual for a 4 year old to complain of anything for any length of time.... Grandmothers have great instinct get him to the GP for a check up.

You can get headaches on your crown - so this is not unusual! If this headache is persistent or frequently re-occurs then I suggest you take him to the doctor.

I would always get a persistant headache checked out. It may be nothing but it's better to be sure in my opinion.

Does he have any other symptoms??? Has he had the headache before??? Is he eating??? Does he have a fever?? Is there any vomiting?? If yes, then call the Doctor, in the meantime try a painkiller for children.

You're obviously concerned so your Grandson's parents should get him checked out.
Possible causes of childhood headaches:
dehydration - is he drinking enough fluid?
eyesight - check his eyesight?
