Sign is objective, tangible information the doctor measures or observes, symptom is subjective non tangible the patient reports or experiences.

Example:
2 patients report shortness of breath, cough and chest pain as symptoms:
Patient 1 -- Doctor measures elevated temperature, crackling lung noise and high white blood cell count as signs of pneumonia.
Patient 2 -- Doctor measure high blood pressure, elevated heart enzymes and abnormal cardiogram as signs of heart attack.
