A bottle of a really fine red wine can be a great 50th birthday gift. A grand cru (the best vinyards in a region) french burgundy wine is probably the easiest choice, but there are so many options open to you.
A camera to record their birthday with.
Personal Stereo. An iPod or MP3 player is great for gardening, exercising or relaxing at home. Here is the thing that you do to make it special. Preload it with songs from their past. That might be well loved classics or pop from 35 years ago when they were first listening to music. Either way, this makes a great 50th birthday gift.
Mini Cactus Garden is a nice idea for someone who never got into gardening, since it takes no effort whatsoever to keep it alive and healthy.
As a really generous gift, why not give a holiday in a country they have never visited? 50 is a great age for travel.
Sports memorabilia are very popular gifts. A tickets from a famous football match or signed team photos perhaps.

At 50, he must have had everything he had ever wanted in his life. What else can there be except things new? But then again, he might hold dear anything that is old fashioned.
I would go for a vintage car. If that is economically not viable, get him a well-shined, antiquated pocket watch.
Best of all, get him a trip to the Old Country...and you need to go along. If brotherly love is of any meaning to both of you, I believe he would want to spend this special trip with his beloved sibling.
How do I know? Because I would if I were either of you...

Is he married? If he is single, a trip to Thailand or some other asia country or Europe.
If he is married, then maybe a trip or something to remind him of something special in the past. A trip to disneyland or hunting trip or a football game or a picture of the family. Something to help feel a bit younger and not so old.

How about taking him out to a nice dinner at an upscale restaurant. Be sure to take a camera to record the memories. Then you could develop and frame some pictures of the fun you had.
