Obviously people are more nervous in interview situations. If you are asked a question you don't know the answer to, you may ask them to restate the question in a different way or provide you with a scenario (if a non factual type of question) so that you can elaborate that way. You can make a brief statement to the effect of "I want to ensure that I understand the question properly so that I can provide an effective/informed answer".
As a recruitment consultant I interview people on a daily basis - I can tell when someone doesn't know the answer to a question - Depending on the question that has been asked I don't necessarily look down on them for not knowing, I will just state it in a different way - but much prefer if someone asks me to clarify the question as it shows that they will seek further understanding, not just say/do what they think and hope for the best.

Sometime during an interview, you may be nervous or you may not understand the question. Ask the interviewer to restate the question. If you still don't know the answer then tell them you are willing to research the answer and get back to them.
After all interviews you should write a Thank you letter. In the letter, you can recap your converstation during the interview. You could also give the answer to the question that they stumped you during the interview.

Hi Missy,
I believe that honesty is the best policy and that you should say...I don't know the answer to that question. May I get back to you after I do some research? If someone asks you a question in an interview that you feel is inappropriate, such as a personal question that you feel awkard answering, try saying...I'm not comfortable answering this question. Will this affect my chances for the job by not anwering? If it's a subject that you know nothing about and don't think you will ever be able to answer just say so...I really don't know. And
leave it at that.

That depends on the type of question. If it is strictly factual you must be honest and say you don't know, but always try to follow-up with a positive statement. Statements that reinforce your willingness to learn, or do more research. Again without specifics I really can't say more, and I am a retired hiring manager.

Well the #1 question is "Why should I hire You?" If you got that figured out then the rest should be easy. Prove that you got something better to offer than anyone else and you'll be fine..or you could always ask them questions before they ask you.

Depends what kind of interview it is.
If it is a finance interview, you need to know your stuff anyway. If you happen to forget something under pressure, you should just talk about something relevant and kind of switch the question on them "unintentionally"
