It all depends on the insult.
Check first whether this deserves a response. You always have the option to ignore the insult & you don't need to bite the bait.
Avoid escalation: sometimes people provoke you & think you deserve punishment, when you pay them back in their own coin. They tend to forget who started it.

A good response can only be given at the moment. An insult "at the ready" will be seen as such, and will not work.
The best way to counter an insult is to turn it around on the other person. I'll refer to the standard, "You're fat." You'd reply, "Yeah, well at least I can lose weight. You'll be ugly forever."
If you're not quick witted, just smile and say thank you whenever anyone tries to insult you. It will really throw them off of their game.

it depends on the situation but i like..
"OMG what happened to you??? were u born like that or were u in an accident"
and..
if ur found looking at someone and they ask "what are u staring at" u reply "i dunno cos i haven't got my animal book!"
