It is slang for ,"What is your name?" in Spanish.
Usually, my Puerto Rican friend use slang more than other Central American, or South American countries.
So my guess is you heard a Puerto Rican person speaking.
It is sort of like speaking to the Improper Bostonian, we will give you an earful of slang! LOL!

Could be used as in Como se llama? Not Como te llama? Como se llama means what is his or her name, but como te llama means what is your name.

Te llama = your name
Se llama= his or her name, but it could also be your name formally
Me llamo= my name

It can mean many things.

It is called
She is called (or named)
He is called (or named)

Exp:

El se llama Luis - His name is Luis/he is called Luis

La cancion se llama "Cielito Lindo" - the song is called "Cielito Lindo"

Vivo en un pueblo que se llama Armeria (this is an actual town in Mexico, lol) - I live in a town called/that is named Armeria.

se llama, come from the infinitivo word llarmarse. It is used when asking about the third singular person. "como se llama" = What's his /her name.

Me llamo jeana= My name is Jeana
Se llama Jeana = Her name is Jeana

It means "you call"

as in "Como se llama" roughly translates to "what do you call?" or "what are you called"? Basically a way of saying "What is your name?"

llama does NOT mean "name".
Nombre means "name". As in "Que es tu nombre?" Means "what is your name?"
