I just bought him a pillow (regular size) I put it underneath his crib sheet so it can not suffocate him. Do you think he is old enough?

What are the advantages of a toddler pillow vs regular pillow?

My son started using a pillow (a regular one) as soon as he moved to his "big boy bed" when he was 13 months. We never had any problems. However, if you're concerned, get one of those vinyl pillow allergin covers. They make the pillow a bit stiffer and harder to smother in. Still, I hardly think a 20 month old is likely to let himself sufficate if he can move around.

If he's still in a crib though, don't do it. There just isn't enough room in a crib for a pillow. He's old enough now though for a toddler bed or a regular one.

A toddler pillow is a bit more safe but I would think he is old enough (I'm a mom on my daughters profile). For now keep the pillow under the sheet and see how he warms up to it and maybe in a month or a few weeks see how he does.

It is old enough. They have to be over a year. Like someone else said, though, it's ok in a bed but I wouldn't use it loose in a cot. You've put it under the sheet so it'll be fine.
Is a toddler pillow smaller?

If he's in a bed a pillow is fine, i wouldnt give him one in a crib.

Toddler pillows are left lofty and smaller, less bulk to get caught in.

I dont know, my 9 month old shares my pillow when we sleep together... I dont know when I'll give him his own.

what makes you feel that he needs one? I don't give my children pillows until they move to a big bed.
