Sentence D. In order to determine the correct pronoun, remove the other name.

"Please give Paul and I your undivided attention" becomes "please give me your undivided attention" so the correct pronoun is "me."

"Jacob and me are going to set a meeting date." becomes "I am going to set a meeting date" so the correct pronoun is "I."

"Carol told Sylvia and he the good news." becomes "Carol told him the good news" so the correct pronoun is "him."

"Send the package to Tanya and him." becomes "Send the package to him." so the sentence is correct.

It's been so long since I thought about grammar, I actually had to look this up. Apparently D is the only sentence which is correct.
