I know employees have to fill out W-4 forms for their federal tax, is there another seperate form they need to fill out for state?

It depends on the state.
Some have separate forms for state and federal withholding, while others just use the federal W-4 form for both.
Effective in Jan. '08, Kansas will start using a separate form (K-4).
If your not sure, find your state's Department of Revenue website and look for withholding certificate.

Depends on how the tax is calculated. If it's a flat rate, or a certain % of federal, then no. In some states you will have to fill out an equivalent form for the state for your withholding.

No, generally if your state has a tax there is no form to fill out as it is a flat rate across the board.

In some states there is a state income tax, and in some there is not.
If you have the state income tax your employer - human resources - should be able to furnish you with the forms and information you need.
