Doctrine and Covenants is a book of "scripture" revealed to Joseph Smith and his successors, which is accepted as cononical by the LDS Church and other Mormon sects. References are separated into sections and verses, much in the same way the Bible is, for ease's sake.
The D&C (common abbreviation) was based on the 1833 Book of Commandments. Most of the Book of Commandments revelations were kept in the new D&C (1835), with the exception of a few very specific prophecies that went unfulfilled, and strange doctrines that were later changed or dropped. Also, many of the existing revelations were changed by Smith. Because most extant copies of the Book of Commandments had been burned (and much of the church's membership left Smith during 1834 and the beginning of 1835), he was able to change them without anyone noticing.

Hey Free Will... You are the first one I met who knew about the Book of Commandments verses the D&C. I find it amazing that the Book of Mormon strongly teaches an unchanging God yet they altered many of the revelations in the Book of Commandments.
