In 35 years of health care I'd have to vote on "burns" being the most painful.
35 years as a combat medic,emt/paramedic,and Operating Room Trauma Tech.

I would say this is an unanswerable question as lets face it no-one on here can be dead and answer truthfully
But I would say Burning alive must hurt a bit
