When a person continually provokes anger or frustration in me, I can come to feel hate. But I will say that that is rare, because I have a high boiling point and it takes me a while to get there. It's also rare because hate can become a destructive emotion in civilized society, even if it served us well (speaking selfishly) in our prehistory... and so I actively try to suppress it and deal with my situation more rationally or constructively.
And what provokes the angry reaction? Anger in all of us is a response to what we sense is ethically wrong. It makes good sense to feel that way because it goads you to take action against what is wrong, thus strengthening the integrity of the society you're in, and therefore securing a better place for yourself in the world. Frustration is a response to having my will or my goals thwarted. It makes sense too, in that it forces me to seek alternative ways of seeing my will satisfied. I can even have this feeling toward inanimate objects if they just get in my way enough, until I say, "I hate this thing!"
All of our emotions are like behavior-shaping directives that increase the likelihood that we will effectively respond to something we experience in life.

Good answers so far. I thought I would suggest my favorite, which is also odd-sounding (which, of course, doesn't mean it's false).
The cause of hatred is love.
This is the answer St. Thomas Aquinas gives to the exact question you ask. In the Summa Theologiae, his mature work, Thomas considers the passions in general as related to the destiny of the human being. The treatment of love features prominently, which is followed by a treatment of hatred. Here I quote his reply to the question:
"Love consists in a certain agreement of the lover with the object loved, while hatred consists in a certain disagreement or dissonance. Now we should consider in each thing, what agrees with it, before that which disagrees: since a thing disagrees with another, through destroying or hindering that which agrees with it. Consequently love must needs precede hatred; and nothing is hated, save through being contrary to a suitable thing which is loved. And hence it is that every hatred is caused by love."
This parallels some of the answers given so far, insofar as the good or will of the agent is frustrated, and that something must be loved or desired (like self, country, etc.) before that which prevents the lover from loving becomes hated.
The rest of the treatment of the question is interesting, especially the discussion that follows regarding 'whether hatred is stronger than love?'. St. Thomas says that it is not.

my evil side is usually conjured especially when i see someone being discriminated in front of many people for stupid reasons, or if i see someone being embarrassed for no apparent reason, or when i see someone abusing their authority onto someone small and cannot answer back because of the stupid social hierarchy. and yes, i am not some kind of a super hero, but you know what, i usually get into physical fight and verbal arguments with those people. they end up suing me. and they will hate me for the rest of their lives while i am relaxing in my bed and laughing at how they looked like when i punched them on the nose..ahaha ^_^
in addition, yes i hate them for that, but hate is something that will never stay in me for the rest of the day..^_^
PS:
in the Philippines, the judicial system is so slow that they rarely pay attention to trivial cases just like what i am into..ahehe, i take it to my advantage..^_^

Hate is an emotion that rears it's ugly head. It cause from something that effects, and contradicts the very core of your exists.
When you realize this is happening, a decision has to be made. Hopefully a good one, that you will not allow this to consume you or define you.

I hate people who want to destroy me, my country, and my way of life just because we exist. It's that knowledge that makes my job writing military fiction books really effective.
