Hurt by Nine Inch Nails and Johnny Cash is very sad, but I think the saddest is Jim Croce's "Time In A Bottle", even moreso because he wrote it for his newborn son and died before he even turned 2, he never did get to spend that time he sung about making it all the more tragic.

EDIT: Almost forgot about "Tears In Heaven", written by Eric Clapton about the death of his 4 year old son and that is definately the saddest song I've ever heard.

I would hope that it would not have the same effect today as it did in 1933 when it was first written but it is also known as the Hungarian suicide song because of the number of people said to have been driven to their death by the haunting lyrics. You can see them in the first source below and the second source tells a little more of the story. Some collections of Billie Holiday songs do include that and it is a great song - unless you are too depressed when you listen to it!!

If you mean sad as in pathetic then it'll have to be the Titanic theme tune but if you mean sad as in weepy then it'll have to be From this moment on by Shania Twain, Alone by Heart, I want to know what love is by Foreigner and Total Eclipse of the Heart by Bonnie Tyler. They always make me cry.
