Yes, a player must touch all three bases and home plate in order for a home run to count. However, the Official Rules of Baseball allows for a pinch runner to do the honors in the event that the batter who hit the home run cannot complete the jog around the bases.

All on-base runners are entitled to advance to home plate and score.
The batter-runner can be pinch-run for since the ball is dead and out of play, and he is entitled to all the bases. The pinch runner will be credited the run scored.
Happened to Gabe Kapler (he was on first base when Graffanino homered) in 2005. Tore some knee or ankle ligaments while rounding second. Trainers tended to him and helped him off, a pinch-runner went in, and completed the trot. Graff had to wait between first and second until the pinch-runner came in.

A pinch runner will be allowed because Bottom of 9th innings my team is leading by 1 run,one million dollars on the line.Opposing team hits a two run shot and the player is executed turning for home.That why a pinch runner is allowed.Referees call.

It still counts.. that sort of thing is accounted for.. just like in foot ball when a player catches the ball but is pushed out of bounds while still in the air the catch counts cause it is to be assumed that he would have landed in bounds. In base ball sometimes great hitters get old or have bad knees so they have designated runners for them.. example I hit the ball but you run for me. if that sort of thing had happen im sure that person would get the honorary base on GP cause he gave his health to entertain and play America's Greatest Game.

Gabe Kapler blew his achillies (sp?) heel while running the bases after a home run. and was allowed a pinch runner.

If a person blows a knee in between 1st and 2nd base and is writhing around in pain, he'd get tagged out. Also, if a guy hits a game-ending homer & he never hits home plate in the course of his team carying him off the field, his run TECHNICALLY wouldn't score (but the ump would have to call that correctly). If he stopped at 2nd, he'd get a double. And if the person was in-between bases, it wouldn't count as a homer...but I don't know if he'd be considered "out."

I think they have to touch all bases 1st 2nd 3rd and home in order for it to count as a homerun.
