Do you mean in acrobat teams like Freccie Tricolori, Red Arrows? well it depends in what direction...forward, side, ....but however few(also 1-2-3 ) meters when the formation is close.
Yes, in big formations like "delta", "diamond", etc, the every airplane is a little higher than the following. they are not all at the same exact altitude to avoid trails(turbolence and exausted gas) of their mates...

As others have pointed out, it depends on the organization flying, the maneuver they are doing, and the point of the maneuver...

In their Diamond Formation, the US Navy's Blue Angels often will have a mere 18-24 inches from wing-tip to canopy (windscreen).

I used to fly in the SH-60B Seahawk helicopter in formation... we'd be overlapping rotor-blades with 8-10 feet of vertical separation.

For most manuvers, the teams are approximately 2 feet apart. There are several reasons for this. It looks better, but it is also easier to fly. The pilot flying on the wing of the lead can see small changes and immediately correct. The farther apart the aircraft, the harder to visualize lead's movements.

Even in training, USAF student pilots fly at 3-4 foot separation. Its not hard to learn but takes lots of practice and concentration.

Military air demonstration teams such as the USAF's Thunderbirds and the US Navy's Blue Angels routinely fly in formations with clearances of only two feet between aircraft.

Depends on the manouver. Lets take the Reds.
In one manouver (i believe its apollo but I wouldnt swear to it) their wing tips really are 4 ft apart. In others, they use crowd position and optical illusions to make it seem more difficult and closer then it really is.
Either way, the reds are the best jet display team in the world!!!
