To begin with, you would be put on a monitered program for as long as a year to 18 months. During that time, attending PT would be mandatory, with the possibility of your having to sign in every session, so failing to go would be disobeying a direct order..not a good thing. More than likely you would also be ordered to talk to a nutritionist (again, not doing so would be failure to obey a direct order) and to come up with a diet (again, it may be monitered with you being required to keep a food log). If you do not make progress, the level of PT will be increased, with regular weigh in's and tapings. If after all this, you continue to fail to meet requirements one of two things will happen...1) you will be denied re-enlistment or 2) you will be discharged. If those happen, you run a very real chance of loosing any benefits you may have accumulated, such as transitional health and access to military commisaries, your GI bill, use of the VA loan and, depending on time in service, it can even take away retirement benefits. Add in that you will have several letters of counseling and at least one Article 15 (formal reprimand) on your file, so working for any government orginization would be extremely tough. If you are unhappy and looking for an easy out from the military, this ain't it! If you are genuinely having trouble with the PT requirments and your weight, the program can be extremely helpful, but it is stressful because you will not be eligable to promote, deploy or PCS while on it. If you want to stay in, put out the extra effort. If you don't, than talk with your command to see if there are other options or just finish your current contract and don't re-up. If you are just lazy or don't like being told what to do, you choose the wrong career field. You'll either need to step up to the challange or accept the consequences.

You will get counseled to lose weight and do better on PT. If you can not do it on your own, then remedial PT and you will go to get your thyroid tested to see if the issue is medical. If that does not work and you fail the APFT or weight, you will be flagged(no awards, special privileges, or schools) and bared from reenlistment. If you continue to fail you will be involuntarily separated from the Army.

The simple answer is that you get your **** kicked. I don't know about the US military, but in the British army you are expected to stay fit and if you don't pass the physical fitness every six months then you are given one chance to retry and if you don't your sent off to a fitness camp. I had a friend that went to one and he said he would rather go to military prison.

Froggy McGurk's answer sounds good to me.
Don't know about the Army, but in the Marine Corps you will be counseled and advised to get fit.
Everyone is required to pass a yearly physical fitness test.
YOU WILL PASS IT.

you will be kicked out for failing to maintain weight and PFT standards. depending on your CoC it could be a general under Honorable or an Administrative discharge. your final months in the military will be miserable as they will make your life very hard for being a lazy slug.

It's getting harder to go unnoticed. You'll get counseled then put in PT till your fit and trim. Dietary needs might get adjusted. Or... You'll run into one of the lifers like me and attitude adjustment will commence.
