Any number of reasons. Having epilepsy rarely leads to surgery by the way.

Most common is a tumor. Even kids can get those.

A list:
Aneurysms of the Brain
Arteriovenous malformations
Cavernous Angiomas
Intracerebral Hemorrhage
Subdural Hematomas
BRAIN TUMORS (malignant)
Adult Brain Tumors
Children's Brain Tumors
Ependymoma
Glioblastoma Multiforme
Malignant Astrocytoma
Medulloblastoma
BRAIN TUMORS (benign)
Acoustic Neuroma
Astrocytoma
Meningioma
Pineal Tumors
Pituitary Tumors
Tumors (other)

I don't think anyone wakes up one morning and decides it would be a good day to have brain surgery or does it on a dare or has a choice of aspirin or brain surgery and decides the surgery would be more interesting. Most likely a doctor told them they really needed to have it done and gave them a good reason. You might try asking them why.
