It causes arteriosclerosis which impairs blood flow to the glomeruli - the microscopic filtering apparatus of the kidneys which function to clear waste products from the blood.
Impairing blood flow leads to poor kidney performance
and eventually renal failure. It's also something of a vicious cycle because progressive kidney dysfunction aggravates hypertension which further damages the small blood vessels.

Long standing HTN can lead to issues with the kidneys as well as visual problems, sexual dysfunction and heart trouble. If you, or someone you know, has HTN (as determined by high readings across a few doc appts) they need some medication as well as dietary and activity changes. Hypertension can kill long before renal issues develop.
