I do 3 sets of bench press, 3 sets of incline & decline press
then I do the ab crunch machine, and that's when I started feeling light headed, but I also felt somewhat weak towards the end of my chest workout, I take effervescent creatine before every workout.

I end my workout with 15 minutes of cardio on the lifecycle bike and I felt like I was going to pass out! is this normal or am I doing something wrong?

yes to answer the water question I do drink water inbetween all my sets.

Bench press is a big compound exercise involving multiple muscles. These muscles necessitate high amounts of energy when working thus requiring lots of oxygen. So yes, it is normal to feel light headed after considerable compound movements.

It depends on when you are working out. If you exercise in the morning before eating breakfast, that could be the problem. I have passed out after exercising in the morning when I did not eat anything beforehand so you should be careful. If that's the case, eat a piece of fruit before you go.

Yes, you don't mention if you drink any water. You should be drinking water, or sipping on water while you exercise. You may be dehydrated.
