no, its happened(like after 9/11) but its rare.
mostly because its too dangerous to by flying fighter jets through heavy traffic commercial airspace.

Not in U.S. airspace with the sole exception of September 11, 2001. But, if flying into hostile air space, most assuredly.
That would include the two Theaters of Operation in Iraq and Afghanistan.
BTW there is only one aircraft in the U.S. military inventory which has landing priority over Air Force One. That is any aircraft accomplishing a medical evacuation mission.

I do not know, but it would make sense, at the very least I am sure there is always at least a few F-16's, F-15s, or today even F-22's throughout the US and the world on alert if something were to come up.
