...From what I saw the left rear tire had been on since about 60 laps to go.
Lap 268 - took 4 tires
Lap 318 - took 2 right tires
Lap 322 - didn't pit (stayed out)
Lap 328 - Jr's tire decides it was faster than the car

From the looks of his pit on lap 318 he took 2 right side tires. Nobody touched the left rear wheel since Lap 268. I noticed on the tape rewind that his crew on the 268 pit stop was on the right side changing his tires.

...So how did the left rear tire fall off on the last lap when it had been on the car for close to 60 laps? That shoud be somewhere around 90 miles on that wheel before it fell off.

Am I missing something?

If all 5 lug nuts were left loose the wheel would have fallen off sooner than 60 laps. We really need to see the axle and wheel. It could have been that 2 or 3 nuts were loose and the others tight. The loose lug nuts would come off soon but the tight ones wouldn't. If there were 2 left eventually the studs would break or the lugs might pull through the wheel. There are lots of possibilities.

Most likely the wheel broke. Junior had reported loose wheels all day on the left side so if it was loose at any time during that run, it could have weakened the strength of the week and just broke the plate right off of it.

Yes, Jr has been saying stuff about wheels/tires on left several time, we had race scanner going..Theres lots of possibiilties here...But again its equipment failure. Hard to believe that something like this can happen...This was so strange... I'm still pissed...He had a great shot at winning again, and the equipment failed.....You have some great theories here Tina and Beth....

Theresa shot it with a high powered lasar from the spotters stand and melted the wheel right off.

Apparently to long., but he didn't have engine problems. That's got to be a step in the right direction.
