Yes. You do require a work permit. You can get the forms from your school's advising office or administrative office.

Under FEDERAL law you need to be sixteen to work in most non farm jobs. Here's what you do. Go to your school district administration office with your parent(s) and have them ask for a Workers Permit Application,have them fill it out and sign it. Then submit it to the address on there. It's usually to your State Employment Commission or State Department of Labor.
Once you get your permit,you'll have to leave a photo copy with every application you make.

No you do not, but most places will not hire you until your 16. I live in kansas, I have 6 kids, and nobody would hire them until they turned 16. My daughter worked at Mcdonalds at the age of 14, I was never asked for a work permit for her.
