In modern biology, six kingdoms are usually used.
(Monera actually isn't used as a Kingdom any more - it's a super-Domain if anything).

Check the source for more information, it'll answer the rest of your questions.

In a five kingdom system, the Kingdom Monera includes all the bacteria. Many use a six kingdom system in which the bacteria are split into two kingdoms. Either way, the other kingdoms are the same.

Kingdom Eubacteria - most bacteria
Kingdom Archaebacteria - bacteria that live in harsh conditions
Kingdom Protista - protozoa, algae, slime molds
Kingdom Fungi - all fungi
Kingdom Plantae - all plants
Kingdom Animalia - all animals

If you use Monera, it takes the place of Eubacteria and Archaebacteria.
