People die, usually in a fast or tragic fashion and they either have unfinished business and don't want to cross over, or they don't realize their dead or they don't know where to go.

Well, many paranormal investigators believe a haunting is attributed to left over psychic energy from the previous inhabitants of a house, school, or what have you. Sometimes, it could be the psychic energy of a living person who is living in that area.
Then, there are past tragedies, such as murders or death by sickness and that people who suffered those things haven't come to terms with the fact that they are dead and so they decide to stick around.
On the other hand, some experts on the occult will generally attribute a haunting to an area where rituals were once performed by people, thereby opening up a doorway to entities, particularly demonic, or as some call them, dark entities...one reason why I don't dabble in the occult as sexy as it might seem.
By the way, some people think hauntings are caused by individuals with certain vices or weakness that spirits use to their advantage by cyphening it from those who they are haunting, making them stronger.
It's a broad subject, but you should do some research on it if you ever have time.

a person or people must die in the house or area. They can just die or die a violent death. If they just die then, they may like the area. Then the person(s) haunt the people that inhabit the area.

Some kind of unfinished business, usually involving a tragic death, takes place, and the resulting left over spirit is trapped in no mans land, restless.

Violent deaths in an area or spirits who just don't want to go towards the light.

Memories become physical things if emotions are involved. Those from horrid trauma (murder, etc) are so powerful that the souls (consciousness) of those who died cannot or will not pull away.
