old address. This person does have a PO Box, though I don't know the actual address. Is it okay to put the persons last known address on the form? Also, should I have it served by certified mail or by a sheriff?

Personal service must first be made and when not possible substituted service through mail and if still not possible, publication on a newspaper of general circulation.

certified mail is fine. that's what I did when I took someone to small claims court and it was acceptable.

i wouldn't use an old address, however. you really need to find someone that has a current return address.

both would work me myself sheriff that way i know he received it anyone can sign and he might use i didnt get it you want have proof sheriff
