Sounds like generalized anxiety with depression. I'm not a therapist but I think you should someone. They can help you undertand your symptoms and help you cope with them. Your confidence is there. You just may need some medication to spark that side of your brain. Get the help. Talk to you family doc and ask for a referral to the mental health clinic. Good Luck!

you are depressed. along with the anxiety attacks which are caused by not being able to confront your fears, which gets you more depressed.
if you are not seeing a psychologist, you should be..... along with treatment of depression and anxiety issues.
you need to take small steps in order to get back to 'normal'
if you see someone you like, tell them 'hi', then when you can do that, then tell them something else like, how pretty they are.
practice talking with older people, they love being able to converse and wont tell you no. once you have that mastered, work on people your own age and go from there.
build your confidence up, one little bit at a time.

It seems to me that you have social anxiety. There are may way to approach a solution for this. The place to start might be to discuss with your doctor and do some research on you own. It really is amazing the amount of information we have at our fingertips. Good luck!
