The clinical spectrum varies from asymptomatic infarction discovered incidentally on radiologic or postmortem studies or at laparotomy for another indication to hemorrhagic shock secondary to massive subcapsular hemorrhage with free rupture into the peritoneal cavity. In other words, symptoms can vary from none to severe.

Approximately one third of splenic infarcts are clinically occult. Meaning there are no symptoms.

The most common presenting symptom is left upper quadrant abdominal pain.

Additional symptoms include fever and chills, nausea and vomiting, pleuritic chest pain, and left shoulder pain (Kehr sign).
