Sounds like severe food poisoning? or Gastric flu? He should make sure he gets rehydrated. If he's throwing everything up including fluids, the main concern is that he'll lose essential salts and minerals. If he's able to keep fluids down, he can still take in nutrition in fluid form, if not, he should get into a hospital.

Since it's been three weeks, we can rule out stomach virus. They don't last that long. That's also a lot of vomiting to be acid reflux. At this point, I would want to rule out bacteria. Have they done a stool culture? Some strains of enteric bacteria can cause symptoms like that.

If that's not it, the next step is investigating digestive disorders, malformations, etc. Just don't let a doctor brush you off. Three weeks of vomiting is serious stuff. Be sure he gets plenty of water. Dehydration is a big concern with vomiting or diarrhea.

It sounds as though you may be suffering from a mental illness known as body dismorphic disorder. It is exceptionally common very rarely diagnosed and surprisingly easy to treat. It comes in varying forms from low self esteem to more serious complications such as anorexia or bulimia. Try to avoid looking in the mirror to much after that first time and try to focus your mind on the positive image you see for the rest of the day. If you do have to look in the mirror after you have eaten repeat to yourself I am beautiful my eyes are deceiving me that is all over and over hopefully over time you may feel more confident every time you look in a mirror. However if you are finding this exceptionally hard to deal with and you are obsessing or considering not eating etc then seek medical advice.
