Looking back on physiology,YES! An extreme pain can activate your sympathetic nervous system leading to "vasovagal shock" which is capable to stop the heart functions that would eventually lead to death. But that can happen very rarely.

What mainly causes death are the main factors or conditions that brings about the PAIN perception. If the cause of the pain has worsened or severe enough, then it will be mainly the cause that can alter various vital processes leading to death.

Pain can also cause death indirectly as those who could no longer bear the pain could sometimes take the option of killing themselves.

I think it could. It depends on how your body reacts to the intense stress and shock of it all. One way your body can try and deal with it is to faint.
