She sounds just right and more attractive than the ultra skinny girls. Now that there have been several people who have answered you she can take a look at these answers and maybe that will help convince her. When I was in my early teens people often thought I was older too, but I was 5'2" and weighed 98lbs so I wasn't fat. They think she is older because she is kind of tall, probably.

Yeah, I think if she's 13 and concerned about being fat, that's bad. She's beginning to think of herself as an individual, not her parent's child. Self-image is a topic that can fill volumes. I'm not sure how you can help her. She seems affected by what other people think of her. This is immature, but happens a lot when you're young.

Some girls grow up sooner. She may be physically mature at 13, though not emotionally ready to be an adult.

Maybe she might believe some "official" person- if she sees a doctor, and the doctor shows her charts for height and weight, she might believe she's "normal."

And if being "normal" isn't enough, the doctor might show her how to improve her diet. (Comment- one thing we don't learn well enough is nutrition. The other is personal finance, but that's an education rant, not an answer here.)

Thats a fine size for her. Judging by her height and how people see her, i would say that she started puberty young. If she is through with puberty, now might be a safe time to start a HEALTHY diet. If she wants to be perfectly fit, then she can find a healthy teen diet. i am 14 and i am a bit shorter and weight the same. i dont see myself as "fat," i just want to stay the same weight so when i am dont with growing, this will be the perfect weight.

If she does not believe you , which is natural, you'd better not try to convince her any more. In such situations like this she would rather trust a stranger than her close friend or her family member. For one or the other reason she will think her close people are just trying to calm her down. There should be some reasons for your cousin to have this exagerated sense of feeling fat , which she is not!!!
