So far, they have paid my bills. The deductibles seem a little ridiculous, though. I've never really thought of "trust" as a part of my relationship with my insurance company.

Say sixish.
Insurance companies are here for the profit, which is a good and bad thing. When there is a drive for profit they can overtax the clients, but at the same time the quality of coverage will go up. The problem with insurance in the united states is not with what we have, but with what we don't have; namely, the many people that cannot afford the coverage. But those that do have coverage typically have decent benefits, good networks and the ability to receive quality care.

I don't trust them at all, and my wife is a physician. It seems that the majority of health insurance claims adjusters simply look for ways to deny payment, for whatever reason they can come up with.
There may be a variety of reasons for denial of payment, ranging from incorrect statement of the procedure by the physician billing department, incorrect reading of coverage by the insurance company staff, and downright desire to not pay.
If something is not covered by your insurance carrier, feel free to contest it and find out why payment was denied. Many times simple corrections and persistence get good results.

About a 5. I have more of a problem with our local hospital. They were caught over-charging. They paid back people without insurance, but those of us who have insurance and still had to pay a lot didn't get a thing.
