A bleeding alcor, wich is stress causing an alcor then not giving yourself enough food and therefore fracturing the alcor which results in vomiting blood.
It could also be lung cancer because that results in fatigue and vomiting. Plus with lung cancer you cough up blood and may also vomit blood.

looks like lung problem - is vomiting in the picture too?
Or it could be ulcer in the intestines - early stage.
or could be an hiv virus - past the early stage.
Better ask the internal doctor for confirmation.
