You can report this to the Wisconsin State Medical Board by going to this website:

Print this out, fill it out and mail it to the board. They should find your complaint interesting. Hope this helps, stay healthy.

Contact a hospital that he practices at and ask for thier "Corporate compliance Number", most hospitals have this as a hotline that you can call and not identify yourself. If you report something serious like this, they are supposed to investigate, and try and fix the problem. Most hospital want to keep thier reputations intact, so they will insist that the Doctor get help.
You can also contact the medical board for your state.

Well, turn him in. People might say you snitched, blah,blah,blah. But, you would be doing something good for him. You would be helping him lose an addiction.Think of it like that and you will be able to do it.

if you are his friend don't call the cops.

no one can help him until he is ready to help himself, calling the police will only dig him in deeper
