PROTEIN. Protein will help strengthen your hair (and also help it grow). You need to eat more eggs/meat/tofu/legumes/dairy.
STRESS is also a known cause of hair loss. You may need to look into some relaxation methods.
DYE. Dyeing your hair too often or using something very strong can cause this. Use treatment on your hair and don't dye it so much!
EATING DISORDER. I don't mean for this to be offensive, but I had a friend with an eating disorder who started losing A LOT of hair. In fact, she practically had a bald patch at the age of 16... make sure you're eating properly..

There are 3 main causes of this. The first is pregnancy. The second is stress. The third is old age. My guess (based on your avatar) is that you are not pregnant nor old. Stress is a very scary thing that affects your body in all the worst ways. Take the time to breathe and participate in calming activities that you enjoy. In the meantime, you can patch up your hair with Jessica Simpson hair. I heard it is really excellent.

You haven't said if you are losing your hair threough it falling out or breaking. If you are not sure, breaking will leave little tufts of hair and generally in a bit of a clump. Falling out will leave you with patches of smooth scalp.
If your hair is falling out please seek professional medical advice, ask your Doctor to take your bloods and rule out any medical problems. Once that has been done, and only after then you can start to look at treatments for hirloss.
If it is breakage you need to give your hair a rest.

It depends on how you are loosing your hair. Assuming you are healthy, it's a natural genetic trend. People have tried various methods of stopping or even reversing it, but that ultimately won't be of much help.
Try to find ways to adapt your hair and clothing style to work for you. A hair stylist will be sympathetic and able to provide some ideas.
It's nothing to be embarrassed about!

That was happening to me along with weight gain and fatigue and it turned out I have thyroid disease.
My hair loss was so bad that I had to brush my hair standing over the garbage can from all the hair that would fall out.
And it was all over my back and falling out all over my arms and was such a mess.
After being diagnosed I have to take a pill everyday and my hair stopped falling out.
Maybe you should consult with a doctor and see it it's a medical problem causing this.

well, that happened to me and i just drank multivitamins.
it actually helps, i have lesser hair fall.
you can also try shampoos for hair fall control.
so there, drink multivitamin tablets and use hair fall control shampoo
