if it's got distemper, it's probably not going to make it. but that's a pretty rare disease to find in a kitten that young, so i'd be surprised if that were actually the case. it would move pretty quickly in a young one with little immune system.

big belly could be from something like worms or other parasites, which could cause diarrhea (not eating good food will do that too), which would in turn make the kitty strain to go potty and make the anus prolapse (or push out).

best thing would be to make a vet appt and take in a stool sample so they can figure out if it has worms and what kind. they're really common in kittens so don't worry too much, just get it taken care of. getting worms (if it has them) under control plus being on good food and in a stable environment will help with diarrhea. sometimes prolapsed anuses need to be operated on to keep them in, but that's in pretty extreme cases. see what the vet says and take it from there.

Take it to a vet. It can have a prolapsed rectum from many things...not just distemper. If it is distemper and it is 3-4 weeks, it will die.
It can be parasites, coccidia, another virus....any number of things.
Get it to a vet for some treatment!!

A vet needs to fix a prolapsed rectum, they know how to do it without tearing the skin.
