Does anyone know if there's a subway/metro that can take you from Disneyland to West Hollywood? I mean, there are underground subway here in Los Angeles County, but would there be one from Orange County back to Los Angeles County?

There is no subway in Orange County. To travel from Disneyland to West Hollywood you would take Metro bus #460 toward Los Angeles, then depending on which part of West Hollywood you are going to would transfer to either Metro bus # 2, # 4 or # 305.

here's one way to this:
go either the Fullerton or Anaheim metrolink station
then take a Metrolink (or Amtrak) to Los Angeles,
then transfer to the Metro 704 direct to West Hollywood (Santa Monica and San Vicente)
and your metrolink ticket would be valid on the 704
this route would be faster and easiest
