They remove some skin from under the eye and then suture it together so the eyelid sits lower and does not curl into the eye.

I had it done for $200 for one eye last year. It varies from vet to vet and depending on the breed, they may make you wait until the dog is somewhat mature before doing the surgery, since the heads on many bigger breeds that are predisposed to entropian continue to grow longer than small breeds.
You can probably call around and get some idea, but the vet would have to see it to give a good estimate. It also can depend on if the upper lid and lower lid are involved.
